1. PLAYERS – Mini-soccer (7 a-side) – Under 7 to Under 10 inc.
All teams MUST have a minimum of 7 players registered by NO LATER THAN 31 JULY 2010 or they may be struck out of the particular division (Rule 8(M) Para 3). Players not registered by 20 AUGUST 2010 will NOT be eligible to play in the first game of the season. (Rule 8(M) Para 2)
Completed Registration Forms together with completed ID Card Inserts should be sent to:-

2. PLAYERS – 11 a-side – Under 11 to Under 17 inc.

REMEMBER Photos MUST be current, i.e. taken this year, last season’s photos will not be accepted. Two copies of PASSPORT SIZED PHOTOS to be submitted with each Registration Form, the boy’s name to be written on the back of the photo and the photos to be attached, not stapled and not stuck, to an ID Card insert. Also please use either a pencil or ordinary biro to write the name on the back of the photo, any other writing means may make the photo unusable because it will not stick to the Card Insert.
Weigh the package at the Post Office and include a STAMPED ENVELOPE, of the same size and to the same amount, ADDRESSED TO THE CLUB SECRETARY. ID CARDS will NOT be sent back to individual Managers.
The ID Cards, together with the blue copy of the Registration Form, will be returned to the Club with the first date the player is eligible to play inserted by the Registration Secretary at the bottom of the form. The Form and the ID Card will both be signed by the Registration Secretary. Any ID Cards NOT signed by the Registration Secretary will NOT BE VALID and cannot be used. If this should occur, please return the Card to the Registration Secretary for verification and signature.
3. CLUB OFFICIALS
CLUB WELFARE OFFICERS to contact either Anita Shaw (Mob. No. 07917 066465) or Simon Shaw, 76 Sussex Road, SOUTHPORT PR9 0SP Tel 01704 539347 about the replacement of ID Cards for Club Officials.
New Clubs to make contact as soon as possible so that ID Cards can be issued before the start of the season. No Club Officials can supervise a team at a match unless they have their ID Card with them (Rule 23) |
